OK.... I'll go for a ground up and see if I can scare everyone with an epic post >.>
- Sketch- OK, I'm gonna be lazy with this part cause I've made this tutorial on sketching based off sketching straight from photographs that's still really applicable to you. This is a tutorial I did for sketching a pencil pic. It has a second tutorial here also. Essentially, sketch out your drawing first- don't go for final lines just yet. Be FAST, and be free with your lines. Get your idea down and have a look at how it looks
For instance, in that tutorial, I have a sketch here-

I've rendered the face alot, but that's cause I scanned it a little further down the track. At that point though, you can flip the image over and see if the image looks correct reversed also. if it doesn't then fix it. Next point- draw them all naked and without hair, then draw the clothing on them and the same with the hair. IT'll help you get the shape right, and know how it falls. You can sorta see the positioning of the arms and stuff through the clothign on that pic.
Lastly- don't be a hero with art- if you want to improve, sometimes a referance is a usefll thing. if you need a kissing pic to help you make it lok believeable, go for it.
- Inking. There's not huge secrets I can give here, except rule your straight lines, and draw all of your curves for the inside of the curve. Draw in 1 long line. You can buy bendable rulers for those really long curves. Make sure you have final lineart down first and that you ink with a pen. Pencil is ok for linearts, but doesn't give a clean finish
- scanning- Scan high res, and shrink later. it's beter to shrink down than start small. lessens the roughness of lines too. Turn your linearts into black and white by threshold if you can, or using levels. Very important so you don't get eh halo effect you have there.
