Technically not a boss,but the Tank from Left 4 Dead.I was walking down a hallway that was so thin....and a Tank comes from BEHIND me,and starts pummeling away at my defenseless self.
Oh,and any Bosses found in the ocean.I don't know why,but any Gigantic Hell-spawn that swims in the water,and can pop up any minute,gives me phobia.

Not all of them, now that I think about it. Only petey piranha, the mantas, glooper blooper, and that worm thing really freaked me out.
But I could never pass the eletric turtle ferris wheel level. I could never get on the ferris wheel at the end to get the star....

Yeah that unused sprite freaked me out. I just think Kefka is just scary because of how insane he is.

The EVIL OF ALL EVIL!I can never beat that. Even with my father. We can always get there most/some of the times though we do get stuck sometime on that one level where you have to ride your bubbles up top. I never understand how to beat him, sure we use those lighting attacks but UGH!
BTW I can't think of any that not from a horror game but Fatal Frame final bosses. It not scary except for there one hit death touch. Get away, GET AWAY..NOOOOOOOO..........game over......<_<

well, okay i kinda do see why but
Though, as a five year old child who watched someone else finish Earthbound, that battle gave me nightmares. When I got a little older (around 8), it pretty much lost its "scary" effect on me. There's a few reasons why it doesn't scare me. One of those reasons is the starting tune in the fight: "Pokey Means Business!" Love the tune (and found the following tunes to be strangely... nice to listen to.) Also, depending on what you input as Ness's name at the beginning of the game, Giygas's dialogue can be rather hilarious (to those with dirty minds...) I also have played/seen others play the game a lot, so I guess that's pretty much the biggest reason why I'm not really affected... I think that I've seen the scene way too much.
Now, as for things that actually scared me... Well, not a boss, but the ReDeads from Ocarina of Time and Majora's Mask really got me. In addition, trying to get through the Shadow Temple (also not a boss) was awful for me.
That game had quite of bit of scary stuff... Well, during the first few times I've played. The ReDeads still bother me, though. The Shadow Temple? Not so much.

The Big Daddies and (now the Big Sister the 2nd one) from BioShock... THEY were scary. Especially when you saw one turn red and charge straight after you when you have no ammo and only the Insect Swarm. I barely killed one because it was low on health and too distracted by the hacked sentries.
As scary as they were, you could pickpocket them when they were dead, so you could get revenge for the extra trip to the bathroom.
