When it does it's normally a result of the camera zooming in, people lose track of their surroundings and miss where the platform is. I've seen it happen to Toon Link's too who try and stab me below them when I pursue them in the air only to miss and die.

If anything I'd say my biggest "Throw" is somehow failing to regain control after running off an edge (either he'll be stuck in his "Skid" animation and die or he won't break out of his spin dash and die).

The main reason why my characters self-destruct sometimes is when I try to narrowly reach the ledge and the game suddenly decides not to give me my double jump.

When I had a session with one of my friends, the lag kept making me fall -- I'd press up-b, but the lag would cause it to register late, and by then I couldn't get back up on the ledge. Luckily, it only happened in that one session.

...Outside of matches where I'm playing Dr Mario for some reason.
